Registration

To Register:

 1. Complete and submit the
    a) Registration Form
    b) Medical Emergency Authorization Form

2. Registration is incomplete if
    (1) USODA Team Trial Deposit is not paid to USODA see www.usoda.org
    (2) Medical Form is not completed and submitted to BBYC
    (3) Registration Form is not completed and submitted to BBYC
    (4) Registration Fee is not submitted to BBYC.
 
3. Registration Fee: $150.00 per competitor if postmarked on or before April 14, 2006. or $200 per competitor if postmarked between April 14 and April 28, 2006.
